Bundling and the National Correct Coding Initiative Edits (NCCI)

In 1996, the Centers for Medicare & Medicaid Services (CMS) introduced the National Correct Coding Initiative (NCCI) to promote uniform claim processing and prevent incorrect payments. This system is primarily used by Medicare Administrative Contractors (MACs), who handle claims on behalf of Medicare. 

The NCCI includes two main components

  • A detailed manual and 
  • A set of coding edits. 

Both are available for free download from the CMS website. However, many find it more practical to use commercial software tools designed to analyze code pairings. It's essential to note that, while many insurers adhere to NCCI guidelines, others employ their coding protocols. Always review the specific billing policies of each insurance provider you work with.

Three Core Types of NCCI Edits You Need to Know

The National Correct Coding Initiative (NCCI) employs three primary types of edits to prevent billing errors and ensure that claims are processed accurately and efficiently. Understanding these categories is essential for compliant and efficient coding.

Mutually Exclusive Services

This category includes procedures that could never be performed together or otherwise cancel each other out. They either conflict with one another or involve techniques that make it impossible to do both. For instance, you could not perform a vaginal hysterectomy and an abdominal hysterectomy on the same patient; it is one or the other. 

Bundled Edits

Bundling edits identifies what services are component codes of other services. Instead of billing each part separately, only the overarching procedure is reimbursed. For example, if a colonoscopy leads to a biopsy, typically, only the colonoscopy code is reported. In cases where procedures are distinctly separate, specific modifiers may be used to bypass the bundling rule and receive payment for each procedure separately.

Medically Unlikely Edits (MUEs)

These edits limit the number of times a particular CPT code can be billed for the same patient on the same day. If the number of reported units exceeds the established threshold, payment will be denied. For example, CPT code 99458 may only be billed up to three times per day. Claims exceeding that amount will not be accepted.
