- Individual psychotherapy (90833–90838)
- Group psychotherapy (90853)
However, it cannot be billed with an evaluation and management (E/M) service unless psychotherapy is also performed that day. It also does not apply to crisis psychotherapy (90839, 90840).
When Should You Use Interactive Complexity?

- Working with young children or patients with developmental delays
- Navigating input from emotional or uncooperative family members

At least one of the following must be present for CPT 90785 to apply:
- Someone else is legally responsible for the patient’s care
- The patient requests a family member, interpreter, or translator to be present
- There’s involvement of an outside agency (e.g., probation officer or school)
- Communication is impacted by maladaptive behaviors or emotional responses
- A sentinel event requires disclosure to a third party (such as in abuse cases)
- Use of play equipment or other specialized methods is needed to engage the patient
- The patient cannot understand or express due to language or developmental issues
