A useful way to evaluate whether an additional E/M is justified is to imagine removing all notes related to the preventive well-woman exam. If the remaining documentation still reflects a substantial and independent assessment, then billing both codes may be appropriate.

For a straightforward renewal without complications, dual reporting is typically not warranted. However, if the patient presents with issues such as breakthrough bleeding, heavier menstrual cycles, or other side effects, and the provider documents discussion, evaluation, and possibly changes the medication, then billing an additional E/M code alongside the preventive service may be justified.
