OB/GYN Medical Billing & Coding Alert
In June 2026, the Kansas Medical Assistance Program (KMAP) made available General Bulletin 26102 containing billing rules that apply to pregnancies that span the years 2026 and 2027, effective July 1, 2026. The policy assumes the AMA will update the maternity code set in the coming years, in line with the CY 2027 changes, which will eliminate all global pregnancy and delivery codes. It explains the process for billing for pregnancy services in this transition period.
It also declares that if a pregnancy ends on or before Dec. 31, 2026, nothing changes. When prenatal care, delivery, and postpartum care all happen in 2026, providers can keep billing the usual global maternity code.(for example, 59400, 59510, 59610 or 59618).
Billing Pregnancy Services That Span 2026 and 2027
Global period will definitely not be permitted when pregnancy services extend across calendar years. Instead, bill services separately for antepartum care, delivery, and postpartum care.
Antepartum Care
For pregnancies that span 2026 and 2027, antepartum care should be billed according to the total number of visits provided by the same provider or provider group:
- 7 or more visits: 59426
- 4 to 6 visits: 59425
- 1 to 3 visits: Appropriate E/M codes
This is when a member delivers before Dec. 31, 2026, and completes postpartum care in 2027, or when delivery is after Jan. 1, 2027 and prenatal care beginning date in 2026.
Delivery Billing for 2026 Deliveries
For members who have delivered after or on Dec. 31, 2026, but provided the postpartum care in 2027, delivery-only codes are to be used.
Applicable delivery codes include:
- 59409 - Vaginal delivery only
- 59514 - Cesarean delivery only
- 59612 - Vaginal delivery after previous cesarean delivery
- 59620 - Cesarean delivery following an attempted VBAC
Delivery-with-postpartum-care codes should not be reported when postpartum services occur in 2027.
CY 2027 Pregnancy Billing Guidance
KMAP states that delivery and postpartum services for calendar year 2027 must be billed according to forthcoming CY 2027 coding guidance. Additional instructions will be published separately before implementation.
The policy is effective July 1, 2026. Implementation by KanCare Managed Care Organizations may vary from the KMAP bulletin effective date.
What This Means for Your Practice
- Flag every KMAP and KanCare patient with an estimated due date after about mid-November 2026. Those pregnancies will not close out in 2026, so they cannot be billed globally.
- Track antepartum visit counts per provider group now; 59425 versus 59426 depends on the total, and the 2027 visits may fall under a different code set.
- The delivery-only code should be used for December 2026 deliveries, and the postpartum claim should be retained until the CY 2027 instructions are published by KMAP. Don't assume 59430 will continue to be applicable.
- Before resubmitting anything to an MCO, check the KanCare Open Claims Resolution Log; each MCO may implement on its own date.
